CARE LINK "Health Care for the Homeless" program provides access to primary health care for the greater Stockton area's homeless population. CARE LINK does the following:
- Provides primary health care services for the homeless at the Gleason House Medical Clinic, 423 South San Joaquin Street, Stockton; all Community Medical Centers, Inc. clinic sites and the Care Link Outreach Program at locations throughout San Joaquin County
- Provides medical outreach services and street outreach to inform homeless individuals of the available services, including entitlement programs
- Provides case management of primary health care needs
- Refers homeless persons for needed specialty care services
- Refers homeless persons for needed mental health and substance abuse services
- Assists homeless individuals in establishing eligibility for housing and entitlement programs
